MHSAA Sports Physical Requirement for 2023-24

Sports Physicals: For the 2023-24 school year, an MHSAA pre-participation sports physical is required for all students who participate in sports. A physical examination must be completed on or after April 15, 2023 to be considered a valid physical for 2023-24. This includes all middle school student-athletes who are participating in school-sponsored athletics. The MHSAA two-page physical form has not changed and remains on MHSAA.com. MHSAA physical cards can be ordered by schools as needed.

Spectrum Health Pennock Sport Physicals 

Spectrum Health Pennock is offering sports physicals this summer by appointment. Spectrum Health Pennock Occupational Medicine will offer a $25 sports physical starting June 1- August 31st, they will offer the discounted rate by appointment only, except on June 14th where they will do walk-in appts. from 8:00am-4:00pm. They ask that if the student is under the age of 18 that a parent/guardian is present with them at their appointment, they also will need to bring in a sports physical form. If they would like to call and schedule an appointment our phone number is 269-948-3102. Their hours are Monday-Friday from 7:30am-5:00pm.
