We are excited to start getting ready for the 2021-22 school year! The information below explains how to proceed with athletic registration for the 2021-22 school year. 

Final Forms Update/Registration 

The TK Athletic Department would like to announce that registration for the 2021-22 school year in Final Forms is ready for updating or new registrations. If you are already registered, make sure to click the “blue box” that is labeled, “Click to Switch to 2021-22 Registration” and you can begin updating the needed information. Make sure to check all the boxes of sports for that calendar year that your student-athlete might potentially plan to try out/participate in so that they can receive information from those program coaches via email throughout the year. Final Forms also has a new feature that allows users to upload a digital copy of their student-athlete’s physical form. The office will always still accept a paper copy but uploading can save time and a trip to the high school. 

MHSAA Sports Physical Update

The MHSAA announced the following message regarding sports physicals on April 16th, 2021:

Sports Physicals: For the 2021-22 school year, an MHSAA pre-participation sports physical is required for all students who participate in sports. A physical examination must be completed on or after April 15, 2021 to be considered a valid physical for 2021-22. The MHSAA Health Questionnaire will not be in use for next school year. Please be sure to inform all parents and students – especially those who will begin middle school next fall – of the April 15 date. The MHSAA two-page physical form has not changed and remains on MHSAA.com. MHSAA physical cards are also located on TKTrojanAthletic.org under the Files and Links section.

Spectrum Health Pennock Sport Physicals 

Spectrum Health Pennock is offering sports physicals this summer by appointment. Spectrum Health Pennock Occupational Medicine will offer a $25 sports physical starting June 1- August 31st, we will offer the discounted rate by appointment only, except on June 14th where we will do walk-in appts. from 8:00am-4:00pm. We asked that if the student is under the age of 18 that a parent/guardian is present with them at their appointment, they also will need to bring in a sports physical form. If they would like to call and schedule an appointment our phone number is 269-948-3102. Our hours are Monday-Friday from 7:30am-5:00pm.

Remember, TKHS fall athletic programs begin their tryouts/practices on Monday, August 9th. Final Forms registration must be completed to participate in tryouts or practices. This includes an updated physical.
